Need a developer experience with website and openLdap

as the title says i want a simple php contact form that users can fill in with a few details. I want to place the file on my webservers, however it must make use of my internal relay smtp server so that must be added, i see many examples of php forms with this online but prefer someone to make me a ready solution for this. Looking to have this done today.

We have an Azure VM running CentOS managed using Plesk with Roundcube as the mail editor. We need to have SMTP forwarding to Sendgrid via their SMPT API on port 587and the necessary DNS records installed/modified. The login data needs putting into a file sasl_passwd on the server so that postfix forwards the outgoing email to the Sendgrid SMTP server, smtp.sendgrid.net.
