5 Low Poly isometric 3d landscape tiles in 3ds (or Blender) with grass on top

For a landscape engine in Unity I need 6 landscape tiles, which resemble a natural landscape / slopes and plains. Works like Sid Meier's Transport Tycoon. See screenshots. I have some (5) templates which I use for the engine. They're very simple. If I stitch them together I can build a 3d isometric landscape. I need these 5 templates with round and smooth transitions from edge to edge so that they look like a bit more natural landscape. Also I want a version of them with grass on top of it. - low poly, not more than 500 polygons per object - should include UVs - should be the size as the original templates - should have some grass on top of it (not more than 10% of the original height) - no textures required. I you provide one with UV, that would be awesome. Project should be accomplished in 2 weeks time.
