FileMaker Pro Database Surgical Case Log

I have FileMaker Pro 11 and FileMaker Go for iPad. I would like to have a database that allows me to enter my patient demographics and notes, document patient admissions to the hospital and surgery center and document all encounters and charges. I use CPT codes and ICD9 codes for all encounters and procedures and need to be able to run reports that shows all procedures done based on CPT, date, and RVU calculations for each month. I hope to be able to use the database on my ipad to add new patients, keep track of all patients in the hospital, do my charges with a printable and email report, and be able to follow my charges by current month, monthly, quarterly and yearly type reports. Need to have area for images and documents for each patient. I have started working on the but have not had the time to finish and would like a bid for this type of project. Would like it optimized for iPad but will also use FileMaker web. Please review my notes file.
