Hi guys and girls! I have an existing plugin which is used to submit a form on my website. It is a phone repair company and the form is used to book repairs. First the customer is asked which device they have. There are radio buttons to select. When they select the device using the radio button the form expands and the next question is asked. This is working perfectly but we are overhauling the site and want to improve this form. The existing form can be found here: [login to view URL] The new form: We want the form to appear inside the image of a phone. The questions will appear as though they are within a messenger application. The radio buttons will be replaced with drop down lists (the existing form already supports this). Each time the customer chooses an item from the drop down box it appears as a reply within the phone. This continues until the form is complete. At the bottom of the phone (either side of the home button) there will be 2 buttons (Go Back & Submit). Go back takes the customer to the previous question. Submit submits the form. (This needs to be greyed out until the form is complete). The new form will ask the same questions but in this order: Model (drop down) Repair (drop down) What is your name? (free text) What is your email address and/or telephone number? (free text) What is your post code? (post code) The next question will appear as the previous is answered. :) The phone needs to be fixed on screen and the contents (i.e. the questions and answers) scrollable within the iphone image. Deadline is 7-10 days. I need the ability to modify the form questions if required.
