Improve Time to first byte (Database optimization)

I am an expirienced system engineer, and familiar with the LAMP stack, to be precise, my current occupation is integration and deployment. Production system are deployed in HA, I use several techniques to implement it depending on customer budget, practices, and existing tool. Then comes the tuning phase to reach response time target. You have already tried few optimization, but the bottleneck seems to persist. I'll put all my experience at your service to find the accurate root cause of this issue, relaying on monitoring data,and impelementing it if ever missing, and fix it, and why not going beyond, integrating cache, minifying, CDN to make the site even faster...
