website application

Simple, hopefully very inexpensive, project. I am interested in posting an application on a website so that applicants for a job respond by entering their information on a questionnaire on the website. The questionnaire is very simple (basic) - see attached - and requests that the applicant fill in some personal information, post resume and photograph, and submit. The appearance should be neat and professional, but does not need to be fancy. The submission should then notify me that the application was received and provide a link that I can click to see all of the information posted, including photograph and resume. It would be preferable if the site can provide me with a real-time report of all respondents so that I can keep a record of my impressions and actions (whether I offered an interview, etc) [In other words, in addition to the individual email/link that I receive, I can also access a report, say a spreadsheet of all of the applicants for final review] I would also like to be able to change what things say, or to add or delete fields, or to make fields mandatory or elective after you have already posted the site without needing to know anything technical. I KNOW NOTHING ABOUT WEBSITES, so this editorial option would need to be idiot-proof. YOU will need to post the website and ensure that it is fully functional at the completion of your task. I will provide you with a web hosting address, but sometimes these sites do not accept international input, in which case, you can provide any free hosting site that you prefer. No graphics or special effects are needed. Just present the application, acknowledge to the applicant that the application was successful, inform me via email that an application was received and provide a link to see the application and uploads. Please review this task and bid appropriately. i will not pursue it if it is too costly. Thank you
