I get you want PayPal's "Subscriptions" integration; this is the program that allows recurring payments from the user to allow access to your website's services. You also want to keep a subscription status in your server, to control such services delivery. I see no problem in setting up a cron job that periodically queries the PayPal platform and updates your replicated users' status. I'm assuming your server will support a cron job written in PHP and making direct CURL calls to the PayPal platform. I would include a run log in that script, to keep a record of executions and events. Assuming you will take care of product and plan configuration as required by PayPal (in the business account interface page), then the only other aspect in your project should be placing the PayPal button in the checkout page. This is easy if you will offer only a subscription plan to your users, but it is more involved if there are several plans. Although system support can be built for handling several types of events that can happen in subscriptions, you didn't mention any of that in the project description and I'm not including it in the current scope.
