GravityForms Ballot Form on WordPress Website

I need a ballot form created, on an existing WordPress site, which is using the Divi theme. I have a GravityForms developer license. So, if you want to use GravityForms for this, all add-ons are available. I am looking for a quick solution, in which you populate the form with data from a spreadsheet. This is a temporary solution, until the actual website we will be using is completed, and live on our server. This needs to be live, by Monday. Do not bid, if you can't do it by then. In your bid, please let me know your plan for completing this project. Please provide 2 bids: 1) Bid, with the thank you message functionality 2) Bid, without the thank you message functionality Users need to be logged in, to vote. Poll results should not be visible to users. I have a spreadsheet of the ballot, with 6,729 ballot items. Ballot items are structured by Category/Sub-Category and Location. The columns, in the spreadsheet are: Category; Sub-Category; Island; Ballot Item; Ballot Code; Thank You Response Message. The ballot codes simply need to be displayed next to their respective ballot items, as they are just there for users to reference (there is voting, on other channels, which requires ballot code entry). I set up an example, using the first category, here: [login to view URL] Here is how the Thank You Response Messages should work: When a user votes for a ballot item, which has a specific Thank You Response Message indicated (on the spreadsheet), a modal dialog should appear, showing the thank you message. If no thank you message is designated, in the spreadsheet, there is a default thank you message. Alternative, if there is not time to do it the way I have explained, above: When the user submits the form, they get a modal dialog, which conains the default thank you message, as well as all of the relevant thank you messages (for the items, for which they have voted). Some nominees (14, I believe) have opted for premium listings, which means I will create a page, just for their business. I need to be able to link any items on the ballot, which have their business name, to their listing page.
