Popover E-mail Sign Up Form Design and Install

I need a popover newsletter signup form designed and installed for my website landing page at: <[login to view URL]>. I want the lightbox functionality where everything grays out and the popover should appear on exit when the visitor tries to click to close the page. I want to stick with the same look and feel of the current signup form you can see in the sidebar of my existing site, but obviously it should be wider and it should incorporate the best practices as detailed at the link here: <[login to view URL]> - inline field label - close button as in the example at link above I want two versions of the headline I can test: - "Get 5 Days of Recipes - FREE During Reader Appreciation" - "Get 5 Days of Delicious Yeast Free Recipes - When You Sign Up Now" Subheader should say: "Don't miss out on several of my favorite yeast free recipes and tips!
