Simple site and design

Hello I need a designer to create an attractive and eye catching design of a website to support testicular cancer. In this context, there must be four buttons on the front end such as home (Danish = Home), Article (Artikel), Forum (Forum), support the association (Støt foreningen), about us (About Us) and contact us (Contact us). Beside or next to the logo must have the following text "Make a difference - donate" with a picture of a bracelet, where it should be possible to click on it to support the association. The under site, Forum, must be the place where people can discuss everything about the cancer. There should also be search bar which do it possible to search after artikel, questions, answer and everything else. In the under site, Article, it must be possible from the CMS to add many articles with integration to Facebook, so your Facebook friends can see that you like the article. In the site there must be a video and 3 boxes which it must be possible to add video/pictures/images from CMS. And there must be a Facebook like box, where the users can see who likes the association. In the under site, Contact us, there must be a contact box where people can send us an e-mail. All the contents must be possible to change on the back end or CMS system. Please see the attached document which is a stripped down version of the page.
