Recover Information

I am in need of a very simple project so I expect the bids to be low and not overpriced. I need a page that my customers can go to recover their username, password or PIN security code. This information is not hosted on the same host so an SQL query that connects to another server will be required. It will work like this: Customer goes to my website ---> Enters Email --> Email is searched in the SQL DB ---> Username belonging to the Email is sent to the same email. If no email is found in the SQL DB --> No account found. To recover the password or PIN number the customer would need the Username and Email. In short, 3 different things need to be recovered using this script and the information must be sent to the account holder's email. It needs to be secured and safe against sql injection and include an anti-spam system captcha? If you need more info just ask but it's a very straight forward project.
