Finishing existing website + Adding Dynamic Form

I need an experienced webdesigner/codder to help us finish up our webpage [login to view URL] as we have been EXTREMELY BUSY and didnt have time to complete our own page. Theres 3 things we need and I estimate that it wont take more than 2-3 hours. 1. We need a contact form/page as the one found on [login to view URL] - On top our contact info which is found on our webpage and then Subject, email, phone, Message field + send button. The CONTACT US LINK on the WHOLE page needs to point to that inner screen pop-up, same as the psd2html.com. 2.I need the section architectural services ready for me just to put in the text and switch images. What i want is really basic. Image attached reflects what i am looking for, but obviously detailed and beautiful. A clear exmaple would be: [login to view URL] 3. Make everything look amazingly pretty, if needed change or add colours, fonts, background(Please!), dividers, css(So it can open properly on any kind of resolution). No childish look !! Professional look only, [login to view URL] is a good example of what professional is for us. We need a designer touch here! This task needs to be completed within the next 10 hours, NO CHILDISH DESIGNS PLEASE, if you are unable to perform the task ON TIME and not able to make the webpage nicer please DO NOT BID. Thanks !
