I need a single page created as a template (read and send examples)

Note: I am hiring immediately if you qualify Read Carefully: Please review the attached screenshot. It is an example of a page I wanted... It is incomplete, but close to what I want. My designer could not accomplish what I wanted, so I need somebody else to create a template page. The site is simple - headline, video, selection box on the right of the video, & 2 testimonials on the bottom The headline and subhead should be text (not an image) so I can change it when I need to (I need a dynamic font, maybe Georgia) The layout should remain like the attachment, shaded nicely next to the boxes and the white against pink. The genie is a gif and she must be standing next to the selection box on the right of the video. **Note: The goal is to get the user to make a selection on the selection box** The genie needs to be larger, bigger than the selection box and moved even, not above (I will provide the gif - she is larger, you will have to reduce her) The callout should be from the genie, set properly. It should say: "Select Your Biggest Problem Area Now!" The selection box Must stand out (Note: On the attached image it is NOT standing out enough) The selection box should PHP coded with those 4 selections Essentially you should replicate what I have with the layout, and then make the changes I want. You should have experience with web 2.0 designs. Big. Simple.
