168647 Form with Upload and AdminSide

Looking for a real simple integration into phpmyformgen or any other form. The form will have basics ie contact name contact number contact email drop down for project type area for l/w/h measurements and a comments section. Also need a button where the user can "browse" their local machine and have a space to upload a digital image. The purpose is to allow the surfer to obtain a free quote. There will be an administration back end so that the site owner can log in and view the pictures the web surfers have uploaded along with their submitted information and get back to them via email with a detailed quote based on the image and info. I will probably provide the form nearly completed via phpmyformgen and need this integrated into it.
