Tweak "Form Maker" Plugin to connect with SQL Server

I have the Form Maker plugin for wordpress which I'm using to capture data from users. This plugin is useful since it saves submissions on the backend which can be filtered and seen/edited. The plugin also has a way to connect to an external mysql database and send the data each time someone fills and sends the form up. I have edited the plugin to also be able to save audio files that are recorded only on the form by the user. The form can be seen here in development server: [login to view URL] All the form fields where set up using only the plugin and the custom audio recording tweak. This form/website will be for a client. The work to be done is this: my client has an SQL Server database where they store all the information concerning this form. They wish the following: the form on Wordpress should connect to their SQL Server database and fecth data from there and present it to the user on the form online. For example, the SQL server database has a field called Cities. The form on Wordpress using form maker should also have a field called cities, but it should fecth (read) the possible fields (New York, Madrid, London etc for example) and list them to the user. They wish to do this since they change the options and fields on this SQL Server database almost daily, and having to edit both sql server and the plugin each time a new change is done would be too much work. So in conclusion: the plugin should read data frmo the sql server and show it on the forms, and then when form is filled save the submission as normal on the wordpress backend but also send it to the sql server database. If you think this is easily doable please bid, I think using the already built-in capability this plugin has to connect to external mysql databases could be used, or maybe a plugin that handles odbc. Also, once you bid please let me know what information (and possible queries/programming) would be needed from my client (database ip, acces to database, etc).
