Email Form not working after site migration (need help)

This is a really small request but need help never the less. I recently had the site [login to view URL] migrated from one host to another and the email form hasn't worked right sense. Not sure what the problem is but needs to be troubleshot so it's 100% again or for the form to be replaced with a new email form script. What it doesn't do? If you enter the information into the form, on a very rare occasion it may send the test email like I got it to do once today however 99% of the time it won't. I'll provide the winner of this job the email it should be going to. Note: The site is nearly entirely designed in php (not HTML)! I don't care what language is used to fix this but wanted you to know this in advance as other freelancers have had some issues not knowing that in advance. Feel free to PM me if you have questions.
