384112 Pre-populate order page

I posted this in December but did not have time to finish, so I'm reposting the job here and would like to start this week. I am running ads and building a list, so the visitors email address and name will be entered into a sign-up form. I'm using Aweber for this. Once the visitor enters the information and hits "submit" they are taken to an order page. I want their name and email address pre-populated on the order page. I will use this for a variety of products. On the Aweber signup form, the visitor enters their "name". On some order pages, they have separate fields for 1st and last names. If first and last names could be filled in, that would be preferable, but it may not be possible. Hopefully, someone already has this code or php file already done. I need instructions on how to install it, where to upload it, etc. Thanks.
