User will enter medical record number a. If “medical record number” is present in database, the current data will be displayed in an ‘editable form' (see [login to view URL]), ie allowing the user to update data. b. If medical record is not in database a new entry is created using the entered “medical record number” and a blank form is displayed (see ‘[login to view URL]') 4. After an entry is submitted the following page is displayed: [login to view URL] As you can see this is a very simplistic interaction between forms and MySQL database. Should be very easy for a seasoned programmer. If you have any specific questions please post to PMB.
