Write an Android application with a web site that can customise the layout and the forms -- 3

Dear Developers I need to create a mobile app in which a manager can create many assessments and customise the forms of each assessment thru a website. The system should be similar to some survey apps out there that allows you to customise and test the surveys thru a website and sync with your app. The assessment contains questions, data entry from the user (Eg, Name and membership number), information screens and the ability to sign at the end. Each question varies by its type, Example: Multi-Select, Single-Select (Radio boxes) that must be available for the user to choose and add to the forms. These assessments must be synced with a backend server and must be able to run offline. Once connected to a wi-fi hotspot or 3g, the app must upload its results to the backend server. These results should be stored in a databases (preferably mySQL) and can be later exported to csv and spss format. The manager should also be able to view the results in various types of graphs. The user of the mobile app must be able to switch between assessments and continue from where they left off even if the app was closed. A side menu should show submitted and incomplete assessments and assessments that have not been uploaded to the server. I am open to cross-platforms. Please let me know if you require any more info. UPDATE: Each assessment requires specific scoring. Most questions will be graded out of 4, there needs to be a review screen at the end of each assessment with the scoring on each question and a total mark of the sum of all questions in percentage. There must be an option to send a PDF of the completed assessment via email to an email address inputted by the user, these email addresses must be saved to the app cache (pop up as a list when typing) so the user does not repeat the process. There should also be an option of quick select assessment, if the user is in an assessment, they must be able to access a side menu in which they can choose a new assessment, the current assessment must then be saved to the incomplete assessments list in which they can access later. Once an assessment is completed and submitted / uploaded the user must be able to access this completed assessment result (PDF) in the submitted assessments list. I do have a wireframe sketch of how I would like the interface to be, this will be available to you once the project is awarded. The orientation of the app must be set in the options menu, landscape or portrait. The splash screen, menu's and assessments will have logo's in the footer section of the app which I will ad later. This is just a baseline of the project, if completed and satisfied as per requirements above, there will be more updates /upgrades that will be required later. Source code must be handed over to me after completion of project.
