Modify website form **READ ENTIRE POST BEFORE BIDDING**

PLEASE READ THE ENTIRE POST AS THERE IS CRITICAL INFORMATION THROUGHOUT! I have a form on my website located at westonmoving.com. There are a few issues and I will need any applicants to address how they would fix this before bidding. If you have examples it would be great. I am not sure what this would cost so I can only afford to do it if the price is reasonable. Here are the requirements: 1. Home page has call to action. The fields on the form need to forward to the long estimate form. 2. The form is very long and hard to follow. If someone had a way to simplify this process and make the form less confusing this would be helpful. 3. The form must work mobile friendly. 4. The form results must only return if a field is populated. 5. (This may be optional) ***SIDE NOTE*** I just rebuilt this website Prior to rebuilding the form fields each had a value and any fields filled out with a quantity was multiplied by the value, then the total of all field values were as a sum and returned in the email to me. I would like this done again. I have all of the code used before if this helps. Once again, send sample work because I will need to make a decision based on price plus the design used. ASK QUESTIONS BEFORE BIDDING IF NEEDED!!!
