I understand that you have problems with setting up goals successfully on Google Analytics and I have a couple of troubleshooting ideas in mind on what could be causing the problem. Your goal type - destination goal - possible resolution: If your page url can already been seen in All Pages report, it could mean setting up the correct regex expression of the url(s) to capture it as a destination or funnel goal. If your order form is not seen in the All Page report, I can assume that your order form is on a different host name so I would need to validate if your order form has being included within your GA profile filters(provided the GA tracking code has been sent to the same property as your main site) My experience includes working on B2B and B2C Google Analytics profiles. I have successfully created event/destination goals on Google Analytics profile and troubleshoot different problems in goal settings configurations. I have Google Analytics debugger installed so I will be able to tell immediately if your page is sending any GA codes to the profiles.
