Create existing document from PDF format to either Microsoft Word or Excel, whichever is better suited. Based on the requirements, it would appear Excel will be better choice. In addition to just simply convert it, will need enhance feature of; 1) Inserting questionnaire of "Yes" & "No" that will populate hidden fill-in boxes upon checking “Yes” 2) Similar questionnaire that will populate hidden fill-in boxes 3) Drop down menu of a full list of “Nationality” of the world There will be 2 set of doc of the above required.

Hi, excel guru here. I did this type of task before at my work. If you need to collect data from respondents using excel file, than I would suggest to go with Google forms as it looks more attractive and it also creates excel spreadsheet for recording responses.
