Create A Custom Report in Magento

I need to create a report in Magento that details the following columns: Order #, Customer #, Company, First Name , Last Name, Address, City, State, Zip, Phone, Date, Item Code, Description, Qty, Item Price, Total Price, Sales Rep, Shipping Cost
Then total qty's at the bottom for items w/ Pricing or Quantities. I'd like the report sortable and filterable for columns such as Sales Rep, Date, City, State, Zip. The ability to export this report in CSV is required.
All this data is available in Magento, I just need it all in one place.
I have an example report in Excel to aid in the construction of the report.
