Port functioning web application to iPad/iPhone

We have a functioning Web application with approximately 20 screens. We use it for our patients to communicate with our physicians. It has an API interface to our billing system so that under appropriate circumstances, a bill can be automatically generated. Our billing system is called AdvancedMD, it is based on a MySQL DB.

We want the EXACT Web application functionality to be ported to the iPhone and iPad in a single code stream. We do NOT want to have to make the same change twice if we add a field. Obviously some of the more complex screens will need to be broken into multiple sections

I have posted one of the screens of average complexity. I do not want everyone in the world to be able to copy our application from posting the whole thing. After I select a winning bidder, you will have 72 hours to review the entire application, during which time you can back out of the bid with no repercussions.

Please ask as MANY questions as you like during the bidding process. I am a former QA manager with over a decade of experience as a director of very large departments at E*Trade and Oracle. I designed this particular application so I know exactly how it works, and what it needs to look like.

Once we are past the 72 hours screen evaluation period and you are actively working on the project, you will need to sign a HIPAA NDA agreement because of American laws regarding patient data confidentiality, this is absolutely non-negotiable.
