Design, Build, Install of Simple Customised Wordpress Website

I URGENTLY need a very simple website to be created with Wordpress and uploaded to the MySQL database I already have installed with my ISP.
There is very little actual content to go on the site at this stage and the functionality is very simple. At this stage I'm really just looking for a very simple attractive appearance and in a few weeks will spend more time expanding the website to make use of Wordpress functionality.
Please see the attached brief which clearly explains what I'm looking for (password is 'website2013').
The successful bidder will demonstrate good design and layout skills. I am only interested in bids supported by examples of work that meet my requirements and if it is not clear that you have read the brief properly I am not interested in your bid.
All work is confidential and 'work for hire' and although the design will be simple I will require you to rapidly produce revisions of the look of the site until I am happy to proceed to the build phase. So if you can work fast and communicate clearly I will be very happy. For such a simple job I prefer working with a single operator rather than a team of people.
