Hi are you able to make this gift card into a 3D version for me?

I have attached a picture '2013' to show you what it looks like in real life.

I am wanting this 3D version to put on my website.

I just want it to be a flat gift card (like how it is sent to you) however corners need to be added & a gloss finish to be applied to a surface so that it looks like plastic. I would like a small shadow to be put around the entire card.

I can supply the gift card image in any file format you need to make it easier.

Can you please let me know if you can do this?
