Simple wordpress plugin created

I need to have a very simple wordpress plugin created. I understand others exist like it, but i need a custom one. I've outlined the details of what i need it to do below. I need it to do EXACTLY what i list, nothing MORE, nothing less. The details are listed below. The plugin should allow me to create a basic form, and when the form is being created, i should be able to attach a numerical value to each field, below i've pasted an example. Are you a boy, or a girl. -Boy -Girl On the back end, i can set a value of 10 for boy, and 20 for girl then when that question is answered, the next question appears. once everything is done, the page will then display a result in a css class that i provide to you using basic math ( addition only ), so each answer is worth a point, and when all the questions have been answered, the total is displayed at the end. but , the user can not see the total until they have completed the questions. you will not need to email the information to anyone, and you will not need to store the answers, just add up the answers to the selections. Budget for this is 30USD, i need it done quickly. Thanks!
