POR002 - Move php script which collects MySQL data to render a FusionCharts object to a new .php file

I have a [login to view URL] page which is part of a Bootstrap3 template.
The page contains the standard header / footer / nav bars etc.
The page displays a FusionChart object with data being sourced from MySQL from a php script contained within the same page.
What I want to do is:
"Move the php script that calls the DB and collects the data to a seperate .php file which is called when data is required for the FusionChart"
Why I am doing this:
The original page will be used to show many charts based on user choices and I do not want thousands of lines of code in a single page.
The source page php script lines that need to be moved are:
/app/[login to view URL] php code - line 396 to line 459
These code lines and the functionality need to be moved to:
/app/modules/performance/[login to view URL] insert in line 23 to 26
The code inserted in the new file needs the to be updated to pass the FusionChart rendering data / functionality back to the original page so the chart is displayed in the original page.
