I need a few php pages. I want an [url removed, login to view], [url removed, login to view] and form.php. The [url removed, login to view] needs to be password protected.
On the [url removed, login to view] a user will be able to edit the nine fields of the visitor page and edit three fields that will populate at the top of the form page. The nine fields are shown in the attachment:

The Plan Holder List will need an upload area to upload a .pdf document. The Order Form will have the three fields that will populate to the top [url removed, login to view] and a field to adjust the Price per Set: area of the form.php.
On the [url removed, login to view] page will be shown in an area no wider that 730 pixels in our website. A user will go to the page and see the content added by the [url removed, login to view] and see a link to a .pdf in the Plan Holder List. As well be able to link to the [url removed, login to view] in the Order Form field.
On the [url removed, login to view] copy the fields that are attached:

With a submit button that will send all information to an email address.
