Email and SMS Delivery

I need a simple HTML/ PHP page design with following 4 fields: 1. Name 2. Date 3. Plan Selected [1,2,3] 4. Email ID 5. Mobile Number FUNCTION In Case Plan 1 is selected : Need to send an email & SMS template on daily basis for next 30 days as and when date is entered. (Ex. The count of the days will be 30 from the date entered). Daily 1 SMS template and 1 Email Template will be sent in 24Hours. In Case Plan 2 is selected: Need to send an email & SMS template on daily basis for next 30 days as and when date is entered. (Ex. The count of the days will be 30 from the date entered). Daily 9 SMS template and 1 Email Template will be sent in 24Hours. In Case Plan 3 is selected: Need to send an email & SMS template on daily basis for next 60 days as and when date is entered. (Ex. The count of the days will be 60 from the date entered). Daily 9 SMS template and 1 Email Template will be sent in 24Hours. In all the three cases the email and sms stops after 30 or 60 days as mentioned. NOTE Already have SMTP Amazon AWS Account for delivering emails. Need to add unsubscribe button in the template to meet the guidelines of Amazon. Also need to check when the count reaches 100-200 users will the email be pushed in a sequence as if all the emails are pushed at the same time it may lead to the failure. So need to make sure that the email is delivered with speed of 1 Email per second for safe delivery. Also need to make the save template page at the bottom of the form before the submit button. Also Each email and sms will carry the name in the form of Hi {%user} UI: Simple entry of fields above with selection of template to be sent which works on when plan is selected. Like plan selection 1 will display templates saved for plan 1 only of both email and sms.
