Create a simple Wordpress plugin

I have a few websites, and probably create more in the future, so I'm looking for a plugin that I can easily add to each site, rather than manually creating the pages on each site.
I'm looking for a wordpress plugin similar to this:
http://www.****[login to view URL] replace **** with "star".
I send the customer an email, and they will click if the service was great, okay, or bad. Depending which they click, it will send them to another page to leave a review. I need it very clean and simple, so it doesn't confuse the customer and make them leave.
In plugin settings I will put the URL where I want them to leave a review, and that's it. I hope I can find someone that will do it for a reasonable price, or I will just do it manually by creating the pages myself.
