Conditional Attachments Form

I need a script for a form that will allow me to: [login to view URL] for name. [login to view URL] for Email [login to view URL] a question with check box answers. (answers= a, b, c, d) 4. Who are you obtaining information on behalf of? (answers= Self, Loved One, Friend, Client) [will determine address and closing paragraph] 4.submit.. Upon submit, User will get an email that is properly addressed to them using their name collected from the form, and address the answer they provided for #3. Along with the custom message that will be sent based upon answer of #3 there will also be specific attachments provided. If user selects the answer of a, they will get the attachment attributed to a. If they select more than one they will get the attachments for each selection specified by admin. Multiple selection will also affect the message, I'd like to be able to customize the message for each selection combination. I also need the answers from the 4th question to determine how I address the inquiry. These messages will be set for each different answer and will precede the other answer/combination of answers in the email.
