Build a Web static survey for an specific campaign

We need to build an static web survey that consists of a welcome screen, 5 steps with 40 questions and a thanks message. The process goes as follows: 1.- We will launch an email campaign to a customer base with a link to a survey (the one we need to build). The link will include Base 64 encrypted data with the customer id to identify who is going to fill in the survey. 2.- After clicking the link, the browser will launch an screen with a welcome message that will pull info from a Data Base using the encrypted id. 3.- The customer will fill the 5 steps. 4.- A thanks screen will appear after completing the process. 5.- An email will be send to the customer who filled in the survey with a generated PDF attached. We also need a Back-end with a login so the user can check the data that was filled in during the survey process. In this Back-end the user will only be given access to read data and download the generated PDF if necessary. We will give you: - UI Design - Access to the Data Base so you can pull and save data - We are the ones who are launching the email campaign so you don't have to worry about it Technical requirements: - Server Side Framework: Laravel - Client Side Framework: Angular - Our Data Base is in MySQL - The Suvey need to be responsive - We used GIT as the VCS - We need you to commit your work daily - We need to get the detail of how many hours you spend on each task so we need a 10 minute daily meeting to check the progress. - The survey is in spanish but we will give you the text content so don't worry if you don't know spanish
