Build of Map upload tool "see description for more details"

Would like a simply tool which we can plug into WordPress where, we upload a CSV with 2 columns, Column 1 would be States Abbreviations, and Column 2 would be counts
Then on a US Map we'd like the States to simply fill in the bubble with the count within Column 2
Additionally, on the map top right "see example" there would need to be a Date Entry field so we might enter the Month and Year the counts are from which would be displayed in this bubble as shown in the attached
