72777 Simple web site mail form

I am moving my site from one server to another. The new site does not support Cold Fusion, which is what is used for my contact us mail form. So, I need a new webform developed. The user would fill in their name, their email address, a subject, and then type in a question. When done, they would click on the send button and the information would be valided (email address exists, subject filled in, main text box filled in) and then emailed to our support ID. They would then be taken to a page that says they their email has been sent. It should work and function like the one I have at my site, add-ins I have a navigation panel on the top of each page and on the left hand side. These need to be retained, with the form in the body of the web page. I have a development site setup on the new server and you will be able to log in and add the files and test. I would need a list of files added so that I can move them over to the commercial site, along with any instructions on special setup requirements.
