php and mysql pages for prepaid card

We need some pages written in php - html and a mysql db for a simple prepaid card program.
These are the main features that we need:
- page for inserting a new customer (fields will be unique id, name, surname, address, phone, etc- );
- possibility to search, look and modify customer details;
- possibility to cancel a customer;
(these details will go in the table "customers")
- We need to assign a card number to each customer
(so you could make another table called "cards" with the fields: card number and the id of customer)
Alternatively, the card number can also be in the previous table "customers" and it is the same as the the unique id. In this case the unique id can't be progressive.
Credits:
you can evaluate how to do this.
each customer starts with a credit = 0,00 euro.
there will be a page for adding a credit to a specific customer or for subtracting a credit to a specific customer.
All the operation of adding a credit or removing a credit must be registered.
At the end of the day the administrator can print a report with all the operations of the day
(for example... today 4 september....
Mr. Mario Rossi charged 20 euro in his card;
Ms. xxxxx spended 10 euro from his card;
Mr. yyyy charged 40 euro;
Total charges of the day: 60 euro
Total spended of the day: 10 euro
For last... when searching for a customer... all the money that he charged and spender must be registered.
There will be a page where we can look for all the operation of charging or spending and the total amount of the card of one customer.
All the pages for searching a customer must be with a form with a text input field for the card number... (the focus will be on that field), because we will write the number there with a barcode scanner...
but we also need the possibility to find a customer by his name, in case the scanner doesn't work or the customer lose his card.

please excuse-me but there is a little changing on the project.
I forgot to tell you that the application will run in local on a computer of our customer, and it will not be in internet.
We also need a table for services.

Then... when a customer spend the money from the card, the operator will not write the amount directly, but will chose the service from a drop down menu.

When the operator charges an amount on a card on the page for charging...
He will not write directly the amount but it will chose an amount from a drop down menu.
And automatically the card will be charged with that amount + the gift.
For example the administrator sets that for 100euro charged there is a gift of 20 euro.
For 50 euro charged there is a gift of 10 euro.
When the operator goes in the page for charging, he will find 100 euro and 50 euro in the drop down menu.
If he choses 100 euro, that card will be charged by 110 euro.
