Contact Form Building - Backend Only as Design is ready

I have an HTML landing page built with jQuery, Bootstrap, HTML5 and Java script and it has a contact form already designed with the necessary buttons, however this is still not functional and need someone to quickly add the code for me to make this work. there should not be much involved in this. My server is running PHP so that can be taken into consideration. The person who will be awarded this project will be given the entire html package and any scripts. I am also asking the person who will be carrying out the change to produce some documentation and identify the changes done and to which files and code line numbers. Thanks
