Form page - PSD to CSS/PHP

Basic form page + thank you page that needs to converted from Photoshop design to PHP site.
There is validation on the webpage. Terms and conditions are represented in a small pop up. The user needs to accept these using a checkbox on the site.
When the user has submitted the information, a confirmation email should be sent to the user and the admin's email address and the user should be forwarded to a "thank you" page.
The page should write a cookie to the users computer, so that some of the fields are auto-filled with their previously entered information, if they use the form again.
The email is a basic text email containing the information.
CSS needs to be W3C validated and cross browser compatible.
IMPORTANT: I need the work done by this monday. DO NOT bid on this project if you cannot meet this deadline.
