Basic Phonegap app using SQLite and Jquery Mobile

I am a web developer, doing my first phonegap mobile app. I am struggling with setting up sqlite. So I am looking for:
Outcome:
1. a basic Phonegap app that has SQLite set up already
2. an example call to a server to get data and insert into the SQLite database.
3. A home page for the app, using Jquery Mobile (no design necessary, just use basic Jquery Mobile example from their docs)
4. A button on the home page, that I can click to execute the SQLite insert
I know Jquery language already, so I need the app to have Jquery Mobile in it.
No need to make it look pretty (do not worry about design at all, treat this as a wireframe, demo or proof of concept).
