I would like a 2 page fillable PDF designed - is a copy of some pages off a website. We would like it to be able to completed electronically as well as printed off and completed manually. There is a section which involves calculation of some numbers. Developing a good functional PDF is top priority, second priority is design elements for it to be consistent with the website brand standards and very professional looking. Screen shots of the website pages we are wanting to turn into the PDF are attached, along with the information on the content for the PDF - a word document and a spreadsheet showing the detail of the calculations. If you would like to do this project - please have examples of interactive PDF's that you have designed that you can show me.
