Simple medical database on microsoft access

I need someone to program a simple database for medical signout information.

It will need to have a formulary for daily data entry and 2 report options.

report 1: as seen on excel table included below. Patient data and current admission data + last 3 days of daily events

Report 2: same as one but with all daily events included

It should contain 3 tables.

Table 1: patient data

Table 2: current admission and procedures (linked to one patient on table 1)

Table 3: daily updates for current admission. (linked to current admission table2)

The printout form should contain data for the last 3 days and fit in 1/2 page landscape format.

The exact data to be included will be provided. I expect approx. 4-6 hours work time and the some for revisions. I need it ASAP. I want it to be in microsoft access.

I included a small excel table with how the printed out report should look like. Yellow fields with "/" are the options selectable from a list. # is a numeric field and the rest are typable text fields.

The POD field should be a calculation of the days that have passed since the last operation.

MR (nummeric 7 digits)

Name: text

Date of birth: text

Original diagnosis: text

Table 2:

Current problems and reason for admission: text

Operation 1: text

Operation 1 date: date

Operation 1 surgeon: option form list (Levitt, Dieffenbach, Wood, Other)

Operation 2: text

Operation 2 date: date

Operation 2 surgeon: option form list (Levitt, Dieffenbach, Wood, Other)

Operation 3: text

Operation 3 date: date

Operation 3 surgeon: option form list (Levitt, Dieffenbach, Wood, Other)

Table 3 and most complicated as seen on all data in the grey fields in the attached excel sheet.

I need it to be programmed fast. If possible today. It shouldn't take too long for and experienced DB programer.
