Adobe LiveCycle Form Development - University Application form for demonstration

I need to present a demonstration PDF (XFA) form to a university based in South Africa. Since it is for demonstration purposes only, the form will have limited functionality, but should be reasonably well prepared for further developments.

Particular requirements include:

* No XSD necessary, I will build the XSD and do the data binding myself when needed.

* No Server-side functionality.

* Form has 12 pages of which 10 pages will contain interactive content.

* Dropdown boxes can be hardcoded with test values.

* 3 User Driven dynamic tables.

* No Javascript validation needed at this time.

* Basic checking for mandatory fields and data types should be included.

* Validation and display patterns for emails, telephone numbers.

Not rocket science, but I'm simply too busy to get to this one :(

## Deliverables

A for-print version of the form is available (and attached). The resulting form should look like the As-Is form whereever possible.

I've also attached the first page of the XDP form as I've started with it, it wouldbe preferable if you can just complete the remaining pages using my form as a start page. Also take note of the naming conventions used (hungarian notation mostly), it would be preferable if you can use something similar.
