Create Website Template from Template

I need a template page created, very similar to the page attached. Actually it could be nearly exactly as attached. I have permission to use the page as template
It's a popup window that needs to be 750 by 550 exactly.
All of the text should be editable, meaning that no text should be graphics. Also the submit button should have editable text as well if possible. Also the main image on the page should be fairly simple, so that I can split test different main images very easily. The page should look sharp and clean.
Don't over complicate this project, as it's very simple. I need the project done today.
Once your bid is accepted I'll provide you with the text for each part of the page, as the text will be different from the template. Also I'll provide the html for the Form submit.

Again it's a very easy project...and doesn't need to be overly complicated. You can add your own professional touches to it though so that it looks nicer than the original.
