An Excel document converted into a fill-able form template with picture & map upload fields

I have an excel spreadsheet that I need converted into a fill-able form template. It needs to be able to operate on both Microsoft & MAC OS and must be able to be used on iPad. It needs to have fields for multiple photos to be selected from the iPad or PC gallery and a field for a map to be added from google maps or similar (both are marked on the attached document) Document must also be able to be saved to device without altering the original template.
