We need a Prestashop 1.5 programmer to write a code that does the following:
1-when Add-To-Cart is selected, a pop-up input screen that allow Customer to key-in information like Serial Number, Notes & e-mail address.
2-these information will be kept in the database for the back office for some processing.
The attached file is the end result of what it should look like.
