Convert Form Data to Word File

I have a current PHP script that takes form input then emails it to an address. I want to keep that same functionality, and add some new features. I have a contract that is a Word Document (.doc). I need values from the form to auto populate fields in the Word document. I then need that document emailed to specified address. Next I have a PDF file that has interactive form fields. I would also like to auto populate values into the PDF. Then the updated PDF should be sent in a third email. So in summary from the FORM on my webpage I need 3 emails sent. 1. The form values in an email 2. A Word Document with auto populated fields 3. A PDF File with auto populated fields, that still retain the ability to interact with the form fields. I have provided the webpage that has the HTML form and the current PHP script that emails the form data. That can pretty much stay the same I just need the Word, and PDF functionality added.
