C# Design Forms

I need few forms (5-6) designed in C#. No coding needed only the form design.
I need the theme of the design to be similar to have the main navigation buttons on the left and the respective content on the right.
This forms are going to be demos for a clinical application system to the content are standard.
## Deliverables
The project is only to design forms with no back-end programming. just 5-6 forms for a demo of a Clinical management system.
the look and feel of the design should be like Microsoft Outlook 2003 and above with the navigation menu on the left side and the perspective pages on the right.
the forms are pretty standard and simple. an example of the forms are :
- Registration form (including a patient personal details)
- Consultation form (including a patient visit details and symptoms)
- Pharmacy form (including a list of drugs, qty, dosage, price)
- Invoice Design form (including an invoice editable list)
- Admin CP-user form (including a list of users and their authorizations)
- Admin CP-account form (including a gridview of the accounts and balances and outstandings)
The controls of each page are common sense (eg. if you are in user form, you should have "Add" "Edit" "Delete" ... ) as if this application is a complete and fully functional application.
more details will be discussed but the designer creativity and understanding of the system is necessary.
