I'd like to create a metrics dashboard in an web environment, ideally based out of an open source like Joomla, Wordpress, etc. It'll tremendously simplify data entry into something other than Excel. (This will also allow better control of the data, but capturing who does entry, who can review the reports, and when updates are made.)

The frontend contains three sections:
- Order Entry
- Dashboards
- Reporting Design
Inputs into Order Entry include two "Sections"
Section 1:
- Group
- Process Stage (Drop down)
- Document Name
- Start Date (Auto captured)
- Complete Date (Auto captured, on flag update)
- Planned Completion Date
- Version Number
- Author (Auto captured)
Section 2:
- A form with up to 100 questions
- Free form field answering a standard question, as needed
- Down down of 1 to 10 for each question
- Time stamp of the questionnaire date
Other Ideal capabilities:
Different access types by user
Ability to add new fields for input
Ability to fields into the first input section
Ability to customize drop down fields
Ability to add questions to the second input form
Standardized graphs and charts highlighting counts, sums, and percentages, averages, and standard deviation
Widget-like customizability in ordering graphs and charts
