Amazing "Contact Form"

Hello.
I need to built an amazing "contact form" with the following requirements:
- Must have two levels
1) First, the user chooses between 5 car options.
2) Then, the user chooses one of the three motors inside each car option... and also fills the blanks ( Name, Lastname, Email, Phone, Comments)
- Must be embedded in an image to get a fancy special effect, something like this:

- Must be visually engaging, appealing, polished and professional.
- Must be able to be seen and used on a website.
- Must be able to be seen on a Mobile device (Just to be seen, I´m not asking for a responsive desing).
- Must be setup to send the forms to Email.
