Create Online Reservation System Using Api Services

I am looking for someone to create a plugin for wordpress that will serve as an online reservation system for car service / limousine business.
The layout and functions that I want are here (90%)

We are moving from RideBits to Limo Anywhere and we would like to have the similar online reservation form as we have now (link above).
** At the moment we only want a quick reservation system with no login/account. **
This will be the phase 1 of this project followed by account login and so on will be the next phase (separate project).
Please only bid if you can complete this within the budget and within a reasonable time-frame. If you think you need 20-30 days to finish this please don't bid.
